Wellola’s Portasana went live at the North East London NHS Foundation Trust in collaboration with The Access Group as part of a first-of-type care model designed to streamline patient flow, support virtual wards, and handle patient discharge from hospital to home. The solution will help the trust meet the NHS England Delivery plan for recovering urgent and emergency services.
At many healthcare trusts, the commissioning process still heavily relies on manual methods, such as phone calls and emails, to arrange appropriate care packages for transitioning patients out of hospitals. This manual approach is responsible for the majority of discharge delays and consumes vital clinical resources for a task that could be automated.

By digitising this process, NELFT can audit the causes of delayed discharge, streamline processes and identify areas for improvement. Patients also benefit from the new system by having clear visibility of their status at all times without having to call or go to the hospital in person.
Umesh Gadhvi, chief digital information officer at North East London, said:
The trust has developed an ambitious digital strategy which has been designed with our patients, and our virtual wards digital pathway enables patient choice of care. The design allows patients to use digital options to access and receive their healthcare. Virtual wards (also known as hospital at home) is a key digital offering to allow patients to get the care they need at home safely and conveniently, rather than being in hospital, therefore supporting hospital avoidance.
Steve Wightman, healthcare director at Access HSC (Health Support and Care), said:
Virtual wards are an exciting opportunity across health and care that sees pressure taken off hospitals by freeing up bed space, and stress taken away from patients by returning them to familiar, comfortable surroundings. With our new platforms, we can support this transition from hospital to home, allowing a greater capacity for care in North East London.
Lyndsey Watson, COO at Wellola, said:
Wellola are delighted to be collaborating with North East London Foundation Trust in partnership with the Access Group to deploy the e-Discharge module of our award-winning Portasana Care Journeys platform for healthcare providers in the NHS. By digitising the critical transition between care providers and supporting virtual wards delivery, Portasana can alleviate the burden on hospitals, freeing up bed space, and empowering clinicians to focus on delivering quality care. This transformative integrated solution paves the way for efficient, patient-centered care in every step of the journey from hospital to home.
